The file appeared roughly at the same time he had attended the hospital for a serious case of amygdalite. However, the file in question has made no mention of the amygdalite. Instead, he said that he had presented chest pain and shortness of breath, attributed to a “probable chest due to coronary disease”. In reality, he had none of these symptoms.

The files, which were examined by Fortune, Also noted that the patient had received a diabetes of type 2 diabetes at the end of last year and was currently on a series of drugs. It also included dosage and administration details for drugs. However, none of these details was correct, according to the patient and several other medical records examined by Fortune.
